4* Do you plan to use all of your vacation time this year?

5*In the event you do not use all of your vacation time, does your employer allow you to carry it over into the next year or do you lose it?

6* When you take a vacation, how often do you check in by phone or email with your office or place of work - Every day or nearly every day, Most days, Some days or rarely or never?

7* How would you rate your boss or supervisor -- Excellent, good, fair or poor? 

8* How would you rate your relationship with your co-workers -- Excellent, good, fair or poor?

9*If you changed jobs, how much would you miss the people you work with – miss them a lot, miss them somewhat, not very much or not at all?
